Our staff is so committed to this endeavor that the Centers for Medicare & Medicaid Services (CMS) ranked our program over and above national averages as we continue to receive recognition for our outstanding service and quality care.

CMS rates agencies using data collected from patients during the admission and discharge process. The measures provide information about patients' physical and mental health, and whether their ability to perform basic daily activities is maintained or improved.

Home health care is important not only for the elderly, but also for younger people whose mobility has been curtailed by operations, accidents or acute illness. Adventist Health's patients range in age from newborn to those over 100 years old. Nurses are available 24 hours a day. SERVICES:

  • Registered Nurses for wound care, diabetic education, medication management, IV administration, ostomy care, enteral feedings, catheter care, respiratory therapy, nutrition counseling, and pain control.
  • Home Health Aides for assistance with activities of daily living such as personal care, bathing, and dressing.
  • Physical Therapists for evaluation of home safety and equipment needs, transfer and gait training, and therapeutic exercises to build strength, improve mobility and decrease pain.
  • Occupational Therapists for gaining independence in self-care, treatment for increasing coordination and dexterity of upper extremities, and evaluation of needed and adaptive equipment to reduce pain and increase mobility.

CRITERIA:

There are certain criteria requirements that are necessary in order to secure home health care services like those provided by Adventist Health. In general, a physician must determine the need for a home health care plan and subsequently, make an order for such care to meet a patient's needs. The physician must also review the plan for care with the agency authorized to perform it.

Among the usual conditions required for home care services are:

  • A doctor's order for home care services for a patient.
  • The patient needs skilled nursing care; physical therapy; occupational therapy; or speech therapy, though not in a full-time capacity. Such care must be performed on an intermittent basis only.
  • If the patient is covered by Medicare, he/she must be determined to be homebound, i.e. not living in a hospital or other long-term care facility, and unable to leave their private residence (home or apartment) without significant effort and only for short periods of time. Private insurance requirements, however, differ in that home care services could be ordered to prevent hospitalization or emergency room visits.

INSURANCE, OTHER FINANCIAL OPTIONS:

  • Think you or your loved one could benefit from Adventist Health Home Care Services, but you don't think insurance will cover it or that you can afford it? Think again.
  • Many private insurance companies as well as Medicaid and Medicare acknowledge the financial benefits of home health care services, and as such, cover many of the services we offer. Whether services are paid for through commercial insurance, public insurance or in some other way, a program can be set up to best meet the needs of a patient.
  • Even with the few exceptions to insurance coverage, there are several other financial options that can be arranged for Adventist Health Home Care Services, including direct payment from a private party - such as the patient, his/her family, or a third party. In these cases, we can even set up a convenient payment plan on a credit card and offer discounts for services and same month billing and payment.
