To complete online pre-registration, you will need:

• Insurance ID card information. 
• Procedure date and time. 
• Type of procedure (such as surgery)
• Personal contact information. 
• First and last name of physician who ordered procedure.

The six-page registration form takes an estimated 10 minutes to complete. (Please note that you will be directed to a separate, secure Web site, and the page will appear different from what you see now.)
